    Please read the following and indicate your agreement by typing your name below.

    Liability Waiver and Membership Agreement

    Before starting (or continuing) your multisport adventure as a team member of Raise the Bar, we ask that you agree to the following. It’s your standard waiver – everybody has them. Thanks for your patience in taking this necessary step:

    I acknowledge that participation Raise the Bar's Multisport Team will entail physical exercise and exertion and thereby have inherent risks. The risks include, but are not limited to, those caused by terrain, facilities, temperature, condition of athletes, equipment, travel, actions of other people including, but not limited to, participants, volunteers, coaches, facility staff, and event monitors, and/or directors of the team and team events. I hereby assume all the risks of participating and/or volunteering as a member.
    I certify that I am sufficiently fit to participate in multisport training and racing, and have not been advised otherwise by a qualified medical person.
    I acknowledge that this Accident Waiver and Release of Liability (AWRL) form will be used by the team managers, sponsors, and organizers in which I may participate and that it will govern my actions and responsibilities while participating in team events and functions.
    As a team member, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

    (A) Waive, Release, and Discharge from any and all liability for my death, disability, personal injury property damage, property theft, or actions of any kind which may hereafter accrue to me or my traveling to and from team activities THE FOLLOWING ENTITIES OR PERSONS: Raise the Bar LLC, their directors, officers, employees, volunteers, representatives, and agents, sponsors, directors, and volunteers.
    (B) Indemnify and Hold Harmless the entities or persons mentioned in the above paragraph from any and all liabilities or claims made by other individuals or entities as a result of my actions during the team activities.
    I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident and /or illness during team activities.
    I understand that during team functions, I may be photographed. I agree to allow my photo, video or film likeness to be used for any legitimate purpose by the team directors, sponsors, organizers, and/or assigns.
    This AWRL shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.


    Raise the Bar Multisport Membership Agreement
     
    Membership Term:   Your membership will be active from May 30 through June 3. This membership entitles you to attend the 2024 training camp! All other membership benefits are reserved for annual members.   If you wish to extend your membership, Contact Patty Swedberg.


    Raise the Bar LLC, Management of the Multisport Team, agrees to support team members in the following ways: 

    -Provide a team structure in which athletes can positively experience multisport and meet their goals for competition and success
    -Provide current, sound, safe training information
    -Communicate as clearly as possible the team objectives and activities on a regular basis
